SCORE-CHD is a local, self-hosted case report form (CRF) and outcomes registry built for congenital heart surgery programs — with built-in STAT, RACHS-1 and Aristotle risk scoring, risk-adjusted analytics, and multi-centre data sharing. The full release is on its way.
SCORE-CHD is a lightweight web application your team runs on its own server or PC. Staff open it in a browser, complete the case report form, and the data is stored locally — no cloud subscription, no per-seat licensing, and no internet connection required.
A local Flask + SQLite application. Install it on a clinic PC or server and access it from any device on your network.
STAT (STS-EACTS), RACHS-1, and Aristotle Basic & Comprehensive categories are auto-calculated as your team enters the case.
